Let's look at an example of an irregularity issue that compromised test validity.  A test examiner begins a testing session, and realizes that the Test Administration Manual and/or Script was not in the materials received for the session.  Instead of requesting the appropriate manual and script, the examiner chooses to allow the students to start the test session without following the script because he feels he has administered the assessment many times over the past few years.
